Complete Self-Certification Form

The self-certification forms (previously one form in English and one form in Spanish) have been combined and are now together on one, single form.

Please complete the entire Self-Certification Form at the beginning of every school day your child is in attendance. 

One survey needs to be completed for each child that is attending school.

Please complete no later than 6:45 a.m.
